Hello and Welcome!

If you follow these few simple steps, you will be on your way to having your first counselling session with me.

STEP 1:

send me an email with your full name & email address.  If your partner is coming with you, their name and email too!

STEP 2:

I will then email you (and your partner, if applicable) intake forms to fill out electronically.  Takes about 10 minutes.  Both of you must complete it individually and entirely.

STEP 3:

once I get the forms, I will send you a link to the calendar to book your first appointment. You are welcome to contact me directly by phone, email or text to book as well if you don't want to go through the calendar.  This Step 3 is ONLY done after step 1 and 2 is done.

When you get the intake forms, please read them carefully, especially regarding insurance reimbursements, cancellation policy, and how payments are handled.

And that's it!  3 easy steps!  Any questions, phone, text or email!

Frequently Asked Questions:

  • Does my husband need to fill out all the forms too?   Yup!  Everyone attending therapy needs to fill out every single one of those forms.  Each of you must log in individually to get to the forms!   I know, seems a lot but each form will take about 2 minutes to go through and sign off.  Think of it this way, filling out forms might be the easiest part of therapy!

  • Why do we both need to provide a credit card?   Simply because each person has their own file, which must contain a credit card authorization.  Also, often in couples therapy sessions are done individually too, as part of the work.  In those cases, whoever attends the session is the one that gets charged.  However, in the intake paperwork you have an option to pay by e-transfer instead of credit card.  If you choose e-transfer, only one of you need to send the e-transfer.  You guys decide who.  Just remember though, the e-transfer is part of your paperwork, so it needs to be sent to me right away.

  • Can I fill out these forms in person instead?   Yes!  However, you do end up using your session time that is booked with me to complete the forms.  So I strongly encourage you to click the green button above and complete them online. 

  • Are these forms secure?   Yes!  I use a great program that follows provincial and national laws regarding security.

  • Okay, I have filled out the forms.  What's next?   Well, if you want to go ahead and see what available time slots there are, you can!  Just click on the Calendar button above.  Or, you can just reach out to me directly to book.  

  • So, I have to pay before I see you?  Yes!  I don't take any in-office payments.  The payments act almost like a deposit.  The intake paperwork explains it all.

  • Do I have to look at the calendar?  Can't I just book directly with you?  Absolutely!  If you don't want to view the openings on the calendar, just reach out to me directly by phone, text or email and I will let you know my availability.

If at any time you have questions about what is on the forms, please stop filling it out and reach out to me.  Best way to reach me is by text.

Please read the forms carefully!  It will avoid possible confusion down the road, especially with regards to the cancellation policy, confidentiality, insurance and fees.
